Hundreds of courts now offer forms at no cost online. You will need an Adobe Acrobat Reader, which you can download for free at www.adobe.com to access most forms. Many are also available in HTML (web-style), Word97 or Word Perfect formats. Many are now interactive allowing you to fill them out on screen and then print out.

Other sites may provide forms which are "zipped." For such files, you will need an "unzipping" program such as WinZip® or PKZip® to uncompress the available files. If you do not have such a program on your computer, visit the WinZip® home page and download an "evaluation copy."


District Court Forms

About half of District Court web sites have a forms section. Forms are also available under Rule 84 or in the appendix, so the local rules should also be checked. Since a majority of the states model their court rules after the federal rules, you can check Civil Rule 84 or the appendix for those states which do.



State Discovery Rules

Our Index of State Discovery Rules provides a list of the civil rules or codes of three-quarters of the states, so this would be a good place to start.  State rules of civil procedure adoption continue to unfold but the index gives details of current adoption of rules in many states.



California W.C. Appeals Board Forms

Many of the California WCAB forms are available at this link. The forms are Microsoft® Word 6.0 documents that have been "zipped" (mentioned above) and may be viewed and printed with Microsoft® Word version 6.0 or higher OR a version of the free Word Viewer (which is made available on that page) and then filled in with a typewriter.


Bankruptcy Forms

Hopefully you will never need to access Bankruptcy Forms, but in the event you do, the Administrative Office of the U.S. Courts has a complete list of the forms available. You can download or print out the entire set or individual forms. To locate the District Court forms, go to the List of District Courts where they provide links to the individual districts.
